Terms & Conditions

ConsidraCare is pleased to offer eligible client’s access to personalized health coaching services through our partnership with NexJ Health Inc. These Terms & Conditions govern your access to and use of the Services. By accessing or using the Services, you agree to be bound by these Terms.

1. Eligibility:

-The Services are available to individuals who are current or past clients of ConsidraCare.
-To determine eligibility, you will be asked to provide information about your status as a ConsidraCare client during the registration process.
-We may request additional information to verify your eligibility.

2. Pilot Program:

-The Services are currently being offered as part of a pilot program for a target duration of 3 months.
-During the pilot program, eligible clients will receive access to the Services at no cost.
-We reserve the right to modify or terminate the pilot program at any time.
-After the pilot program, the availability and cost of the Services may change.

3. Service Description:

-The Services provide personalized health coaching to help you manage chronic conditions and improve your overall well-being.
-The Services are designed to complement the care you are already receiving from ConsidraCare.

-Two levels of service are available:
Message-based coaching: Daily or weekly communication with a certified health coach through messaging for personalized guidance and support.
1-on-1 video or phone coaching sessions: Personalized coaching sessions with a certified health or cancer coach.

4. Client Responsibilities:

-You are responsible for providing accurate and complete information during the registration process and throughout your use of the Services.
-You are responsible for actively engaging with your health coach and completing any assigned tasks or activities.
-You understand that the Services are not a substitute for professional medical advice, diagnosis, or treatment.
-You should always consult with your physician or other qualified healthcare professional for any health concerns.

5. Data Privacy & Security:

-We respect your privacy and are committed to protecting your personal information.
-Our use and disclosure of your personal information is governed by our Privacy Policy.
-NexJ Health also has its own privacy policy that governs its use of your information.
-You consent to the collection, use, and disclosure of your personal information by us and NexJ Health in accordance with our respective privacy policies.

6. Liability & Disclaimers:

-The Services are provided "as is" and without warranty of any kind, express or implied.
-We do not guarantee specific outcomes or results from the use of the Services.
-Health coaches are not medical professionals and do not provide medical advice.
-You agree that we and NexJ Health are not liable for any damages arising out of or relating to your use of the Services.

7. Termination & Cancellation:

-We may terminate or suspend your access to the Services at any time for any reason, including but not limited to your breach of these Terms.
-You may cancel your use of the Services at any time by contacting us.

8. Governing Law:

These Terms shall be governed by and construed in accordance with the laws of the Province of Ontario and the laws of Canada applicable therein.

9. Entire Agreement:

These Terms constitute the entire agreement between you and us relating to your access to and use of the Services and supersede all prior or contemporaneous communications, representations, or agreements, whether oral or written.
